Thrive Hubs

Who are Thrive Hubs?

Thrive Hubs are a single point of entry, front door approach to the emotional wellbeing and mental health offer for young people aged 5-18. Our service is designed to advise, guide, signpost and get help for young people within Manchester currently facing mental health and emotional wellbeing issues.

The Thrive team will work alongside community-based services not traditionally acknowledged for their role in the improvement of good mental health and wellbeing, to enhance their offer. The aspiration is that young people and families will no longer be removed from their community, schools or colleges when seeking help for their emotional health and well-being.

 

 

 

 

Types of service we offer

M-Thrive is a low-level intervention service, operating within the ‘getting advice’ and ‘getting help’ sections of the thrive framework quadrant. Our service is designed to advise, guide, signpost and get help for young people within Manchester

Our Vision

Our vision is for Manchester to be a place where young people feel empowered. We want our young people to feel confident in reaching out for support in times where they may be struggling.

Our Mission

Our mission is to ensure all young people in Manchester can get the desired support they require, whether that is through engaging a young person in some activities within the local community, providing 1-1 sessions or linking them with specialised services.

Our Purpose

Our purpose is for every young person to be able to feel accountable and manage their mental health and wellbeing, and not let it take its toll in difficult situations. Through different strategies we aim to inspire and motivate our young people to be the best version of themselves possible.

Our People

Our team is made up of pathfinders, navigators and practitioners who all liaise with the relevant schools and partners and offer different levels of support.

Pathfinder

  • Understanding the child’s needs
  • Sign posting – guiding young people to the next steps
  • Liaising with other services, as well as schools & Parents/Carers
  • Low level emotional health & wellbeing support such as emotional regulation sessions to Primary schools

Navigator

  • Understanding the child’s needs
  • Offer up to x3 sessions around emotional health and wellbeing & building resilience
  • Liaise with other services/schools around implementing strategies of support & supporting Parents/Carers
  • Onward referrals to EH/Specialist services

Practitioner

  • Understanding the child’s needs
  • Offer up to x6 sessions exploring around “getting help” – this might look like local offers
  • Liaise with other services/schools around implementing strategies of support & supporting Parents/Carers
  • Early Help/CAMHS Consultations
  • Training to Stakeholders

Thrive Hubs Referral System

As a single point of entry, access to our service comes via a referral form. The referral forms can be completed by parents/carers, schools, general practitioners. A self-referral form can also be completed via the self-access form on our website. If a young person engages with our service and gets their desired outcome, however further down the line a new issue presents itself, the option to re-refer into the service is always available.

Any organisation can refer a child or young person aged between 5-18 (or 25 years if the individual is on EHCP). Self-referrals are also accepted

Our Manchester M-Thrive | Home acts as a digital front door. This is a single Emotional Health and Well-Being Directory offering a wealth of resources and signposting to services, organisations and activities that are aligned to resources to assist children and young people and  their families, services and partners

Frequently Asked Questions

You can visit a Thrive Hub in person, or you (or your parent/carer) can fill in a form to get support.

  • Ages 5–8: Parents, carers, or schools can help children this age come to the Hub. We can give advice, talk about worries, and sometimes run group sessions.
  • Ages 8–18: Young people can come on their own for help and advice.
  • Ages 18–25: Young adults with an Education, Health and Care Plan (EHCP) can also use the Hubs.

No. Thrive Hubs don’t give medical diagnoses. We help early on to stop problems from getting bigger.
If a diagnosis might help, we can tell you where to go and who can do it.

We meet in different places depending on what works best for you.
Most sessions are at your local Hub, but we can sometimes meet at school, a youth centre, or even at home.
We’ll always check with you and your family first to decide the best place.

Before any sessions, we ask for your permission to share information.
Sometimes, we need to tell other people who help children and young people so everyone can make sure you’re safe and get the right support.
We only share what’s needed and always keep your information private and safe.
