We work closely with a wide network of community colleagues including GPs, Active Case Managers,\u00a0<\/span>Hospital at Home, Crisis Response, Falls Teams, Social Services, Macmillan teams, and partners across the voluntary sector – ensuring every patient receives seamless, joined\u2011up care.<\/p>\n

Your development matters to us. We support our Band 5 nurses to complete their Specialist Practitioner Qualification (District Nursing) – preparing them to progress into District Nursing Sister roles.<\/p>\n
