Who are we?

We are a team made up of paediatric nurses, play specialist, support workers, counsellor and community paediatrician, providing a range of home and community support for children aged 0-19 years who are resident in Manchester and have life Limiting or life threatening conditions.

The service is a part of the Children’s Community Nursing Team and works closely with voluntary and other statutory agencies. We are known to our parents and their carers as the STAR Team.

Our Aims

To provide holistic, family-centred care for children with complex health needs at all stages of their illness.

To co-ordinate and undertake their care provision within the home/community setting, thus aiming to minimise the need for re-admission to hospital

To enhance the quality of life for children and families by alleviating the stressful events which a family may experience caring for a child with a life limiting illness

How to make a referral

The Star Team has an open referral policy, accepting referrals from parents, carers and professional care workers across all agencies.

The nurses will make an initial home visit to discuss and plan with the child and family an individual package of care
