We are a team of children's nurses with extensive community training and experience.

Our areas of expertise include chronic disease specialists (Epilepsy, Asthma, Diabetes, CF), Continence Team, Palliative care Team and Home Ventilation/Continuing Care Team.

We support GPs with the acutely unwell child by offering home support, observation monitoring.

Generic Support – early discharge support following planned or unplanned hospital admission.

Our areas of expertise include chronic disease specialists (Epilepsy, Asthma, Diabetes, Cystic Fibrosis), Continence, Palliative care and home ventilation/complex care.

We support GPs with the acutely unwell child by offering home support, observation monitoring.

We offer generic support – early discharge support following planned or unplanned hospital admission, dressing and wound care, intravenous antibiotics at home, insulin for newly-diagnosed diabetics, oxygen saturation monitoring.

We operate appointment clinics in health centres across Manchester – general nursing, constipation, eczema, asthma, phlebotomy.

We have chronic disease specialists including epilepsy, asthma, diabetes, Cystic Fibrosis, a Palliative Care Team, a Home Ventilation/Complex Care Team and a Special Needs School Nursing service.

We work closely with the disabled children’s team (social services).

Location:

The team are based at Longsight Health Centre. Clinics are run at various health centres across the city.

Hours:

8.00 am – 10.00 pm, 7 days a week. This service will offer guaranteed nursing triage within 4 hours of receipt of referral (within office hours).

Referrals:

Referrals are accepted by email, telephone (24 hour answerphone), or post. Referrals are accepted from:

  • Walk in centres, A&E, O&A units
  • Lead Professionals of Multi-Agency teams
  • GPs
  • Colleagues, fellow professionals, voluntary, statutory and non-statutory organisations
  • Individuals and families known to the team
  • Hospital and Community Paediatricians

We will see any child or young adult under the age of 16yrs (25yrs if they have complex/special needs). Please complete the CCS1 Children and Young People’s Service referral form (found on this page), fill out all mandatory sections and forward to the team at – mft.manchesterccnt@nhs.net

If you wish to refer a child for a blood test, please use the CCS1 Children and Young People’s Service Referral Form and PSD Ametop Form (for topical anaesthesia) and forward to the team at mft.manchesterccnt@nhs.net
