Intermediate care is a service that is designed to rehabilitate you with input from a team of dedicated professionals. It is a service which promotes faster recovery from illness, prevents unnecessary hospital admissions, supports timely discharges and maximises independent living.

What does the service provide?

The intermediate care team will work with you to set achievable goals and plan your care to enable you to become as independent as possible. They will devise a Rehabilitation programme to suit your needs. Where possible you will be encouraged to participate in activities which you would normally do at home such as making your bed and preparing your meals, dressing and undressing and attending to personal hygiene.
