Who is the service for?
Any patient with a life limiting illness approaching the end of their life with palliative care needs – all patients referred should be registered with a GP in North Manchester. Patients/carers and professionals are able to refer into the service making it more accessible for patients.
What does the service provide?
We provide emotional support for patients and carers, support to manage symptoms and complex needs. We can signpost you to volunteer and advisory services
Who will patients meet while under your department/service’s care?
Patients will see a Specialist Macmillan Professional who have undertaken specialist training to care for patients and relatives who have advancing cancer or other life limiting disease. Within the team there is a Consultant in Palliative Care, a Gp with specialist palliative care interest. Specialist Nurses, Physiotherapist, Occupational Therapists, Speech and language therapist and dietician. We also have assistant practitioners and a volunteer coordinator.
When is the service available?
Monday to Sunday – 8am – 8pm including Bank holidays

Therapists will visit patients in their own homes.
