We work closely with GPs, local authorities, voluntary organisations and other healthcare providers.

We provide care in your own home and other locations including nursing homes, health clinics, Withington Community Hospital and GP surgeries to make sure you get healthcare which is coordinated and meets your needs. We provide services locally, so you get the support and care that fits around you.

Our staff include community nurses, physiotherapists, occupational therapists, speech and language therapists and many more.

What We Do

We provide services for adults to keep you healthy, and support you to make positive lifestyle choices and manage your long-term conditions to help you avoid going into hospital. Help is available to increase exercise, eat healthily, quit smoking, and assist you with your wider health and social needs.

Our nursing and therapy teams provide care in people’s homes and help in managing long-term conditions, so you don’t have to go into hospital unnecessarily. If you do need inpatient care, we will support you to get back to home as soon as possible by providing rehabilitation at home or in one of our Intermediate Care centres.

Putting our patients first

We want you to have a positive experience of our services, be treated with dignity and respect, and be shown care and compassion. You should be given clear information about the care you receive, including in a different language or format if you need it.

You should be involved in decisions about your care and treatment and supported to manage some areas of your own health, where appropriate. Our aim is to provide safe, high quality services. Please talk to your health care professionals about any concerns you may have.
