Discharge Services

A service for adult patients who are expected to be unable to meet essential daily living needs when they are ready to leave hospital and require an essential service in the community in order to facilitate discharge from hospital, including support for patients who are in a terminal stage of illness.


These services may include:

  • NHS Continuing Health Care
  • Intermediate Care
  • Social Services Care
  • District Nursing Services
  • End of life care
  • Signposting to housing and homelessness services

Any patients who require any health and/or social services to facilitate discharge from hospital.
