Our Philosophy

The Supportive and Palliative Care Team provides individualised holistic support including but not limited to physical, psychosocial, emotional and spiritual care. Working in partnership with other teams, support is provided based on an individual’s needs irrespective of diagnosis and at any point along disease trajectory. We consistently strive to improve the experience of patients, relatives, carers and staff by seeking new, innovative and effective means of care delivery and health promotion. Central to this is the provision and role modelling of practice that matches the best available evidence whilst also contributing to this through active participation in the quality improvement agenda.

In-patient Support

The Team provide support to all adult inpatient areas across the Oxford Road Campus

We operate a seven day service (including bank holidays) between 09:00 – 17:00.

Healthcare Professionals from other organisations can contact the team directly on 0161 276 8958 to discuss referrals

There is a weekly Supportive & Palliative Care Team MDT Meeting every Tuesday between 09:00 – 11:00 at the Oxford Road Campus

Out-patient Clinics

There are two Palliative Medicine outpatient clinics that are held weekly. These are currently virtual clinics run by Palliative Medicine consultants for patients who need support with managing their physical, emotional and social concerns relating to their long term condition(s).

Referrals are accepted from the ward team, GP’s and other healthcare professionals. Each patient will be assessed on an individual basis and a plan of care established.
