The Community Medicines Optimisation Services is a team of clinical pharmacists and technicians who work in the community and are based at the Universal Square Office.

Our team is responsible for medicines optimisation within the Trust’s community settings. These include:

  • Adult and Specialist Community Services (including Intermediate Care at Gorton Parks and Community Adult Learning Disability Service)
  • Children’s Community Services
  • MFT Community Clinics

Our work involves carrying out audits to support the safe handling of medicines, medicines reconciliation, medicines management on transfer of care, and the security of medicines and Controlled Drugs.

Our team is responsible for the production and review of MFT Patient Group Directions (PGDs), guidelines and policies, governance support, Non-Medical Prescribing (NMP) and medicines information queries. We also cascade information regarding medicines alerts and recalls to Community Services staff. Non-medical Prescribing within MFT in Central Manchester is led in three distinct groups: Central, Community Services and Trafford. The NMP Lead for Community Services is Jaiesh Lad, based with the Community Medicines Optimisation Service. Jaiesh can be contacted for information about training to become a prescriber, registering as a prescriber and access to prescribing data by e-mail (Jaiesh.Lad@mft.nhs.uk ) or phone.

The Community Medicines Optimisation Service is also involved in providing services that aim to collaborate with other healthcare providers in primary and secondary care, and pioneer new ways of delivering healthcare, medicines management and medicines optimisation to patients.
