Personalised Cancer Care
Our cancer patients are supported by our Cancer Nurse Specialists (CNS) and Macmillan Cancer Care and Support Coordinators.
Patients are offered a holistic needs assessment (HNA). This helps identify concerns and worries a patient may have.
Patients will be referred to support services and community nursing teams if needed.
The aim is for all patients to be given an end of treatment summary. This will explain their follow up plan and any signs/symptoms to look out for.
Follow up including Personalised Stratified Follow up (PSFU)
All patients are seen in the out-patients departments for a face to face clinic appointment to discuss their diagnosis and treatment options.
Once treatment is completed patients are followed up either by:
1) Consultant led clinics
2) Nurse led clinics (usually over the telephone)
