Supportive & Palliative Care Team

We support individuals, families and carers who are affected by life-limiting conditions either as inpatients across Oxford Road Campus of MFT or via our out-patient clinic. Our multidisciplinary team specialise in the management of physical symptoms as well as providing emotional, psychological and holistic support. We work closely with other clinical teams across organisations and can provide a link to additional services including Chaplaincy & Spiritual Support Teams, Welfare Rights, Community Macmillan Teams, Volunteer Groups and Hospice services (in-patient and day-care). We link closely with our colleagues in the local community in particular in Central and North Manchester with these teams meeting with us on at least a weekly basis in our MDTs.

Our Philosophy

The Supportive and Palliative Care Team provides individualised holistic support including but not limited to physical, psychosocial, emotional and spiritual care. Working in partnership with other teams support is provided based on an individual’s needs irrespective of diagnosis and at any point along disease trajectory. We consistently strive to improve the experience of patients, relatives, carers and staff by seeking new, innovative and effective means of care delivery and health promotion. Central to this is the provision and role modelling of practice that matches the best available evidence whilst also contributing to this through active participation in research projects.

In-patient Support

The Team provide support to all adult inpatient areas across Oxford Road Campus including the Manchester Royal Infirmary.

We operate a seven day service (including bank holidays) between 09:00 – 17:00.

The Supportive & Palliative Care Team can be involved at any stage of the patient’s illness.

Staff at the Trust should refer patients and/or carers, with their agreement.

Healthcare Professionals from other organisations can contact the team directly on 0161 2768958 to discuss referrals

There is a weekly Supportive & Palliative Care Team MDT Meeting every Tuesday between 09:00 – 11:00 at the Oxford Road Campus.

This guides effective management planning by discussing patients who are new to the service and have been reviewed by a member of the team.

Attendees include core team members as well as input from local Community Macmillan Palliative Care Teams, a Pharmacist and a representative from the Chaplaincy & Spiritual Support Team.

Out-patient Clinics

The Supportive and Palliative Care Team provides an outpatient service  to all patients who need support with managing physical, psychological, and social concerns related to their long term condition regardless of diagnosis. The clinic runs 3 times a week and is led by both Palliative Medicine Consultants and an Advanced Nurse Practitioner.  Referrals are accepted from the ward team, GP’s and other healthcare professionals. Each patient will be assessed on an individual basis and a plan of care established.
