Diabetes (NMGH)

Who is the service for?

Inpatient (IP) and outpatient (OP) services for people with diabetes related complications, diagnosis of diabetes and on going support/treatment for complex diabetes care such as co-morbidities, feet and dietetics input. The service accepts GP and Practice nurse referrals for OP care, GP referrals and emergency support via A&E for diabetic complications.

What does the service provide?

The service provides one to one support with a Consultant Diabetologists, Diabetes Specialist Nurse, podiatrists or dietetics, there is also an established multi disciplinary team for complex.

The service provides the following appointment types:

  • MDT
  • Consultant
  • DSN
  • Podiatry
  • Dietetics

  • Dieticians
  • Podiatrists
  • Members of the MDT

When is the service available?

Monday – Friday 9am to 5pm with extended services for young people and transition.

Where is the service provided?

Rochdale has an outpatients service only. At all other sites we have an outpatinets and inpatients service.

Practical information

Patients should bring a copy of current medication to all appointments.
