Cardiac Rehabilitation is a structured programme of care and support from hospital admission to return of daily life. The aim of the service is to educate you to understand your individual risk factors and to influence favorably the underlying cause of cardiovascular disease, and support you in maintaining a healthy lifestyle.
Cardiac Rehabilitation can be summarised as shown below.
- The inpatient stage or after a “step change” in the patient’s cardiac condition.
- The early post-discharge period.
- Structured exercise training with continuing educational and psychological support and advice on risk factors.
- Long term maintenance or physical activity and lifestyle change
The CMFT Cardiac Rehabilitation Service is made up of a multidisciplinary team of highly skilled specialist nurses, physiotherapists, occupational therapists and Rehabilitation Assistants.
By participating in Cardiac Rehabilitation, you will see that you may by your own efforts, preserve or resume your optimal functioning, and through improved health behavior, you may slow or reverse the progression of disease.
