Personalised Cancer Care

All patients are offered an HNA (Holistic Need Assessment). Our Cancer Care Coordinator will arrange a suitable time with the patient to:

  • identify concerns
  • start a conversation about needs
  • develop a Personalised Care and Support Plan
  • share the right information, at the right times
  • signpost to relevant services

At the end of their treatment patients will be given a treatment summary. This tells them about their follow up plan. It also gives advice on health and wellbeing, and signs and symptoms to watch out for.

Follow up care

After Treatment a patients follow up plan will be detailed in the End of Treatment Summary.

This will be managed by

  • Nurse Led Clinic
  • Consultant clinic
  • Referral back to their Primary Team

Radiological imaging and blood tests will also be part of the follow up care.
