If you live local to MRI you will be contacted at home by the Cardiac Rehabilitation team. If you live out of the local area, you will be contacted by your local cardiac rehabilitation provider. If you have not heard from your local provider within four weeks after discharge, please ring 0161 276 6048 and we will look into this for you.
The cardiac rehabilitation multidisciplinary team is comprised of highly specialised, experienced and skilled staff who are competent to provide rehabilitation to all patient groups.
If you have any questions or concerns regarding your heart condition once you have been discharged home, please telephone us on the above telephone number between 08.30 and 16.00 Monday to Friday.
