  • Deactivation of Unverified Accounts Associated with Duplicate Records

Unfortunately, a problem has been identified with your MyMFT account – it hasn’t been matched to an existing healthcare record that we hold. This may be because some of the data you entered when signing up didn’t match our records (e.g. your date of birth, surname or NHS number), some of your information may have changed since we last saw you (e.g. address or email address) or that we don’t hold an active health record for you at the Trust.

To prevent future problems we will need to deactivate your account – which we plan to do on 3rd July 2023. After this date, if you wish to sign up to match with your health record and view this information in MyMFT please then follow the steps outlined below.

We are sorry for the inconvenience that this may cause, but please be assured that we are only doing this to ensure you can have ongoing access to your future healthcare information.

Signing up for proxy access

If you are trying to sign up to gain proxy access to a dependent who is receiving treatment at the trust, e.g. in the case of an under 16 who can’t sign up themselves, you will first need to set up a MyMFT account yourself. If you do not have active account or are unable to successfully set this up please speak to the healthcare team at your dependent’s next appointment who will be able to set this up for you.

Please also note that an Under 16 can’t sign up directly online, if you are trying to do this you will also need to speak to the clinical team providing treatment who will be able to assist.

Create a new MyMFT account without an Activation Code via Internet Browser

1. Navigate to my.mft.nhs.uk

2.Under “New User?”, select “Sign up now”

 

 

 

 

 

 

 

 

3. Under “No Activation Code?”, select “Sign up online”

 

 

 

4. Complete the ‘Sign up for MyMFT’ form.

Please note, you will need the last 4 digits of your NHS number. For support in find this, please use the URL above the sign up form.

 

 

 

 

 

 

 

 

 

5. Select “Next” and proceed with verifying your email address and creating your log in details.

Please note, we have removed the username from any unverified accounts so you should be able to sign up using your old one.

Reactivate your MFT account without an Activation Code via the MyChart App

1. To download the app, search for “MyChart” in your mobile App Store

 

 

 

 

2. In the list of organisation, select “Manchester University NHS Foundation Trust”

 

 

 

3. Select ‘Sign up’.

 

 

 

 

 

 

 

4. Next to “No Activation Code?”, select “Sign up online”

 

 

 

 

5. Complete the ‘Sign up for MyMFT’ form.

6. Select “Next” and proceed with verifying your email address and creating your log in details.
