In November 2024, concerns were raised around incomplete or missing outpatient documentation in the service. In line with national guidance, a comprehensive patient safety review was undertaken, involving independent external experts.
The Niche Consultancy has also completed an independent governance review, which MFT commissioned as part of its ongoing commitment to continuous learning and patient safety. A comprehensive action plan has been developed in response to the governance review, with a significant number of actions already underway.
