Update

We are currently undertaking a review of our outpatient waiting lists across the Trust at this time. We have begun sending text messages and letters out to patients asking them if they still require treatment and would like to remain on our waiting list.

This page contains both easy-read and translated versions of these communications, which can be accessed by using the links below.

It is important that you respond to these messages within 14 days of receiving them.

If you need to get in contact with us, please use either the email address or telephone number provided on your original letter.

We are undertaking a review of patients on the outpatient waiting list and would like to understand if you still need an appointment.

To confirm your choice, please complete our online form, the link to the form can be found at the end of this message.

Please complete the form within 14 days, even if it is to confirm that you want to remain on our waiting list.

If you would like to discuss this with a member of the team, then please contact the department on [telephone number] or email us at [email address].

If you have already received a date for your appointment, it is important that you still attend.

Please note; if you are on more than one waiting list you may receive this letter more than once. It is important that you respond to each one separately.
