Following diagnosis of a heart condition that requires medical or surgical treatment you are admitted to hospital and treated with evidence based care.
You are given a plan of care, which is also documented in the medical and nursing notes. The ward staff will support you in the early recovery period, answering specific questions which you may have and provide information which includes:
- What is heart disease, angina, or a heart attack
- Investigations and treatments which have been undertaken
- What are my medications for? How to take/use them.
Prior to discharge the ward staff electronically refer you to the cardiac rehabilitation service most local or appropriate to you. You are contacted within a few days after discharge by the Cardiac Rehabilitation team service you were referred to and appointments and plans for secondary prevention will be discussed.
Part of the recovery plan may include referral on to other services e.g. dietitian.
