The MFT Excellence Awards recognise deserving staff and teams who work at Manchester University NHS Foundation Trust’s nine hospitals and community services.
The Patient Choice Award is given to a person or team who has provided extraordinary patient care – demonstrating a genuine interest in patients, their dignity and care.
Only members of the public and patients can make a nomination for this award. You can nominate any staff member working at MFT who has provided care for yourself or someone you know. A patient representative will be on the shortlisting panel for this award.
Please allow enough time to complete and submit your nomination by the closing date on Thursday 28th October.
