Get help with signing up for MyMFT
To download the MyMFT app search “MyChart” in your app store and select MyMFT.
As a MyMFT user, you can:
- View MFT appointment details
- Attend online video consultations
- Keep track of important test results, letters and health information in one place
- Save time by checking-in online and by providing key information ahead of appointments
- Use proxy access to oversee a family member or dependent’s healthcare (after receiving their consent to do so!).

To make a proxy access request for someone under 16, please fill out the Under 16s Proxy Access form.
With ‘Single Sign On’, you can now access MyMFT using your NHS App details. Find out more, at our Using MyMFT & the NHS App together page.
