The multi-disciplinary Specialist Palliative Care team supports patients with complex palliative and terminal care needs, and their carers, to enable patients to die in their preferred place free from pain and other unwanted symptoms.
They work closely with other healthcare professionals such as GPs, District Nurses, Consultants and Social workers to assess people’s needs and provide specialist support where required.
Many patients with cancer or other life threatening disease might not require this high level support from the Specialist Palliative Care Team but they will be supported by generic services e.g. District Nursing who have excellent working relationships with the Specialist Palliative Care team.
They also provide education, training, advice and support to other professionals on symptom control and end of life care.
