This service offers patients with a confirmed diagnosis of Parkinson’s/parkinsonism.
The service provides clinical monitoring, medication review/advice, a continuing point of support / contact and where appropriate referrals are made to other relevant services and aim to prevent hospital admissions, promote early discharge and reduce time needed by consultants to review patients.
