The Urgent Care Therapy Team receives referrals for patients living at home who have experienced a rapid onset of deterioration in their functional ability that might result in imminent patient injury, carer breakdown or a potential admission to hospital.
A therapy assesment will take place at the patients home by a qualified occupational therapist and/or physiotherapist to identify patients needs.
The team operates within the hours of 7.30am-5.30pm, Monday to Friday.
The team consists of occupational therapists, physiotherapist, patient support worker and assistant practitioners.
The team are part of the Community Enhanced Care Service and work closely with the community matrons.
The team are happy to receive referrals from any professional person and aim to assess these patients within 1-3 working days dependant upon the need identified.
