We have started sending letters out to patients who are currently waiting for a follow-up appointment and have been waiting more than one year asking them:-
- Can you wait for an appointment to become free or do you need to be seen sooner because of your health needs?
- Do you no longer need an appointment and wish to be removed from the waiting list?

We are writing to you as you are currently waiting for a follow-up appointment within [Specialty], at [Hospital site] and have been waiting more than one year.
Here at Manchester University NHS Foundation Trust (MFT), we are giving you the opportunity to contact us to arrange an appointment if you are having symptoms suggesting your condition has worsened.
This letter is asking you two questions:
- Can you wait for an appointment to become free or do you need to be seen sooner because of your health needs?
- Do you no longer need an appointment and wish to be removed from the waiting list?
- Do you need to be seen sooner?
If your health has got worse, we would like to offer you a patient initiated follow up (PIFU) appointment that will let you speak to a member of the clinical team to determine if you need to be seen sooner. Please call [phone number] between 8am and 4pm and we will try to offer you an appointment as soon as we can. It may still take up to 12 weeks for your appointment.
If your health has not got worse and you would still like to wait, you will get an appointment when one becomes available.
If you have already got an appointment, it is important that you attend. If you have any questions please call [phone number] between 8am and 4pm.

If you no longer need an appointment and want to be removed from the waiting list please leave a message by calling us on [phone number for mailbox] and saying:
“My name is ___________and my hospital number is [populate MRN]. I wish to be removed from the waiting list for [populate specialty] , waiting list ID [populate with waiting list ID] because (please choose one of the following):
- My symptoms have gone
- I no longer want an appointment
- I have moved and you are no longer my local hospital
- I have received treatment (or I am waiting) with another NHS hospital
- I have received treatment (or I am waiting) with a private hospital
If you are on one or more waiting lists you may get this letter more than once. You should respond to each letter.
