The Community Rehabilitation service sees patients in their own homes and provides a comprehensive assessment of rehabilitation need in order to develop a patient centred rehabilitation programme to enable the patients to gain maximum independence with activites of daily living and mobility.
The rehabilitation programme may run for up to six weeks, managed by occupational therapists, physiotherapists and experienced support workers. There are 4 locality based teams and an outpatient rehabilitation team.
