Requesting a Test
Single Gene & Panel Tests
All requests for single gene and panel tests should be made using our standard test request forms. The form should be filled in completely paying special attention to the R code of the test and the including full phenotypic details of the patient and family history details where relevant.
Please ensure that the relevant proformas are completed for Immunology, Deafness (R67 Non-Syndromic Hearing Loss) and Neurology (R56, R57, R58, R60, R62, R78) testing.
Whole Genome Sequencing
Many indications have now moved to Whole Genome Sequencing (WGS) in line with the new National Genomic Test Directory. For all such tests, please use our NW GLH Whole Genome Sequencing Test Request Form.
